Judgmental or unprofessional comments ? Example: “Patient is difficult” or “non-compliant” without context

Mischaracterization of your statements ? Example: You asked questions about treatment, chart says “refused treatment”

Outdated Information

Resolved conditions still listed as active ? Example: “Active: Pneumonia” from 2 years ago that fully resolved
