HIPAA Notice

This Notice describes how medical information may be used and disclosed and how you can get access to this information. It is intended for situations where PatientLead Health LLC receives or maintains protected health information (“PHI”) in connection with services you request.

If you are unsure whether HIPAA applies to your interaction with PatientLead, contact us. Some services may involve sensitive information even when HIPAA does not apply.

Our responsibilities

  • We maintain safeguards designed to protect PHI when we handle it.
  • We limit uses and disclosures to what is necessary for the purposes described in this Notice, unless you authorize otherwise or the law permits or requires a disclosure.
  • We will follow the terms of this Notice for PHI handled under this Notice.

How we may use and disclose PHI

We may use and disclose PHI in the following ways, as applicable to the services you request:

  • Service delivery. To provide and support the advocacy, documentation, or coordination services you request.
  • With your authorization. To share information with family members, caregivers, clinicians, or other third parties when you ask us to do so.
  • Operations. For internal operations such as quality improvement, training, and security.
  • Legal and safety. When required by law, or when needed to address serious threats to health or safety, consistent with applicable law.

Your rights regarding PHI

Depending on the context and applicable law, you may have the right to:

  • Request access to PHI we maintain about you.
  • Request corrections or amendments to PHI.
  • Request limits on certain uses or disclosures.
  • Request confidential communications by alternative means or at alternative locations.
  • Request an accounting of certain disclosures.
  • Receive a copy of this Notice.

Authorizations

When we need your permission to use or disclose PHI, we will ask for written authorization. You may revoke an authorization in writing, except to the extent we already acted based on it.

Complaints

If you believe your privacy rights have been violated, you may contact us to file a complaint. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
