Who We Are

PatientLead Health LLC is a single-member LLC operated by Carrie Schluter, BCPA. Services include health advocacy, case review, communication support, and patient education for individuals with complex and invisible illnesses.

Our Responsibilities

  • Maintain the privacy and security of your protected health information (PHI)
  • Provide you with this notice of our legal duties and privacy practices
  • Inform you if a breach of your unsecured PHI occurs
  • Follow the duties and privacy practices described in this notice

How We May Use and Disclose Your Information

We may use or disclose your PHI:

  • To communicate with you about your care via secure methods (IntakeQ, encrypted email with consent)
  • To obtain payment for services rendered
  • To comply with legal obligations, such as court orders or law enforcement inquiries
  • With your written permission for any other purpose

We will never:

  • Sell your information
  • Share your information for marketing purposes without consent
  • Share your information with employers or unrelated third parties

Your Rights

You have the right to:

  • Request an electronic or paper copy of your records
  • Ask us to correct information you believe is incorrect
  • Request confidential communications
  • Ask us to limit what we use or share (we will comply when possible)
  • Receive a list of those with whom we’ve shared your information
  • Receive a copy of this notice at any time
  • File a complaint if you believe your rights have been violated

Communication & Consent

We provide services primarily through the secure client portal (IntakeQ). Email (via ProtonMail) and video sessions may be used with your consent. Your preferences will be documented in your intake form.
