Terms & Disclaimers
By using this website or subscribing to Navigator programs from PatientLead Health LLC (“PatientLead,” “we,” “us”), you agree to these Terms. If you do not agree, do not use the website or programs.
Scope of services
PatientLead Health provides condition-specific Navigator programs delivered by subscription. These programs offer education, decision frameworks, documentation guidance, and system navigation support to help you move through complex care with greater clarity and structure.
We do not provide emergency services. If you believe you have an emergency, call 911 or go to the nearest emergency department.
No medical advice
Information on this website and within Navigator programs is for educational purposes and general support. It is not medical advice, diagnosis, or treatment, and it is not a substitute for care from a licensed clinician.
No legal advice
PatientLead Health does not provide legal advice. If you need legal guidance, consult a licensed attorney in your jurisdiction.
No provider-patient relationship
Use of this website or Navigator programs does not create a provider-patient relationship. Any clinical decisions remain between you and your licensed healthcare professionals.
User responsibilities
- You are responsible for the accuracy of information you provide.
- You are responsible for decisions you make based on Navigator programs or materials.
- You agree to use Navigator programs in good faith and not for unlawful purposes.
Data storage and export
Navigator programs are designed to preserve user privacy by storing program entries and tool outputs locally in your browser rather than on PatientLead Health servers. Your entries remain on the device and browser you use to access Navigator unless you choose to export them.
Navigator includes a Data Manager tool that allows you to export your program data to a downloadable file. Exporting creates a durable copy that you control and may store securely for future reference or restoration.
Because Navigator does not maintain server-side copies of your program entries, you are responsible for exporting and safeguarding your data. Clearing your browser data, changing devices, using private browsing modes, or certain browser updates may remove locally stored information.
PatientLead Health does not have access to, and does not retain, copies of your Navigator entries. We cannot recover locally stored data that has been removed from your device.
Subscriptions, cancellations, and refunds
Navigator programs are delivered by subscription over six months. Subscription terms, cancellation policies, and refund conditions are provided at the time of purchase. Digital content and program access are generally non-refundable once the subscription begins and materials are accessed.
For specific questions about your subscription status, billing, or cancellation, contact us using the information below.
Testimonials and outcomes
Testimonials reflect individual experiences and do not guarantee results. Outcomes vary based on individual circumstances, provider practices, and system constraints.
Intellectual property
Website content, Navigator program materials, tools, and frameworks are owned by PatientLead Health LLC or licensed to us. You may not reproduce, distribute, or resell materials without written permission. Personal use of materials within your own care is permitted as part of your subscription.
Limitation of liability
To the extent permitted by law, PatientLead Health LLC is not liable for indirect, incidental, special, consequential, or punitive damages arising from use of the website, Navigator programs, or materials.
Governing law
These Terms are governed by the laws of the jurisdiction where PatientLead Health LLC is organized, without regard to conflict of law rules. Replace this with your preferred state and venue language.
